Why is this code not adding to the database when I am trying to register?

This is the code and what I am trying to do here is to register the name to users table. When it is registered, it is supposed to add to my table but it is not adding. It is only showing the message that shows register successful but the credentials are not added to the database. How do i fix it? I am using local database btw
